The patient will have to share their Personal Health Record [1] which is stored elsewhere or in your example been captured by a device with the doctor. 

They are several PHR systems out there, including but on limited to Indivo [2] and Microsoft Health Vault [3] 

may be what we would like to see is the ability to easily share data between the PHR's and GNU Health.
