[Bug-gnubg] Minor rollout setting patches

The enclosed patch addresses the following:

1) Bug - the selection for when to switch to later evaluations was
   being ignored

2) Feature - added checkbox (default on) to say that truncation point
   evaluations are the same as player 0 initial evaluations

3) Feature - default is now that rollout settings for players are the same

4) Cleanup - removed unused items from rolloutpagegeneral struct
