I would opt for storing both too. For a lot of reasons, not the least medicolegal ones. Same with examination findings etc.: they will be captured both in a full text narrative as well as in the respective normalized tables. This will cater for unusual situations (not limiting the doctor in what and how he documents events/findings) and will make information retrieval a lot faster.
