[Gnumed-devel] bootstrapping db system

This now goes to the point of creating the core database
"gnumed". It stops short of importing any SQL schema or
setting up services.

It'd be rather helpful to hear some suggestions as to how to
proceed from here particularly on how to handle schema
imports: do them directly in Python from conf files, do them
by invoking psql, ... ?
