Just to increase general confusion, one more suggestion (idea comes from the 
original GEHR project):

Each service gets a unique ID, like an URL. Gnumed.net will soon set up an 
XML-RPC server that allows registration of services.
Whenever data is imported/exported, it gets tagged with this ID (lus the usual 
path of timestamp/author/location 

That way, you can easily exchange health records without losing the track, 
always being able to backtrace information.
