[Gnumed-devel] EMR structure (episode/encounter/...) again

BTW, while working on the allergies table(s) I revised the
episode+friends tables to match our current understanding as
discussed here and in the forum on gnumed.org. Can someone
please take a look (Tony, Gavin, Sam, etc) ? It's in
gmclinical.sql.

In the next few weeks I will review what we got and put a
description in the todo for the developer's manual -- unless
someone else volunteers.
