[Gnumed-devel] gmMeasurement/remote foreign keys

Hi all,

remote foreign key support (well, recording them) has been
added to the standard bootstrap files. Public database is
updated accordingly. Cron script for actually checking for FK
violations still pending.

Same goes for gmMeasurement which has been added to the
clinical service, had an overhaul to fit with the
clin_root_item/episodes/etc concept and has a few example
remote foreign keys set up. It is now in a form that seems
suitable to support nearly any type of measurement/test result
(Christian, no support for linking to normcurves yet but your
ideas have been worked in - normcurve definitions belong into
the service "reference" anyways, I guess).

Someone please review gmMeasurements.sql for egregrious errors
and suitability for PIT path results.
