It gets even better. Here we refer to a speciality. We don't
specify either a target doctor nor a target organisation. And
we aren't supposed to give advice to the patient on which
doctor to choose. Which rule is happily ignored daily,
however.

Maybe it is feasible to have the user select "refer to
doctor" vs. "refer to organisation" via some radiobutton ? I
know that's a bit ugly, though.
