Well, the database is already set to *something* (back from
when the question was asked first). You can find out to what
by running
select * from i18n.curr_lang where user = current_user();
I just added a menu item for (re)-setting it from the GNUmed
client ;-)

It proably should install a system-wide configuration file
as /etc/gnumed/gnumed-client.conf -- there is an example in
client/doc/gnumed-client.conf.example.
