If show/hide triangles are put in place for all health issues (even  
when they had no episodes or encounters) this would indeed better  
permit them to be seen (interpreted) as being at the same (health  
issue) level of the tree as the other health issues...

... hopefully it is ok that unexpanded items may invite clicking,  
only to show nothing inside (??)

I am developing an idea about past history items, operations etc that  
may have a bearing on the above...
