That's up to a template and doesn't concern what I meant to
ask. I was thinking about the *structural* fields needed
only, not how to present them. Or at least only presenting
them to the doctor.

Sure, that't why there's a comment field. It's more of a
matter of *how many* columns we will need in 99% of cases.
