When corresponding with another clinician outside the praxis (e.g. in a 
referral) it can be helpful to include one or more external ids. A helpful 
placeholder would:

- allow to designate (specify) a single external id by

        issuer=, name=, null='user specifiable string'

e.g. issuer='BC.CA_VCHA', name='MRN Medical Record Number',null='not available'

returnable as

        issuer || < – > || name || <space>  || : || value

        BC.CA_VCHA – MRN Medical Record Number: 912345678

and maybe even

        issuer=*, name=*, null='user specifiable string'

could return all as an item list?
