[Gnumed-devel] Workflow of Adding medication / non-medication substance intake

Presently the flow is:

- input brand (or substance + preparation)

- decide about when it was started (but in production it often defaults 
correctly)

- skip the discontinued field

- complete the
        schedule
        duration
        episode <-- required
        aim
        advice

What if we instead (see two screenshots)

- retain the positions of {Brand … (down through) Preparation}, inclusive BUT

- follow this instead with the episode (which is required) followed by

        option 1:               Aim / Advice (because these relate naturally to 
the episode, and objective of treatment)
                                then
                                Schedule and Duration (below-adjacent to find 
the start and discontinue dates)

        option 2:               Schedule and Duration
                                then
                                Aim / Advice
