[Gnumed-devel] Medications plug-in, refactored - Medication List manager

The following is an important background question informing any Medication
management:

=====================================================================
1. between what historical dates *did* a patient take a substance or dosage of
medication?
=====================================================================

Comment 1a:
------------------
Cases which did not extend to now(), despite that they form no part of a
Current Medication List, can be of clinical interest, and are fully capable to
be entered into GNUmed 1.3.8, and -- even if these did "extend to now()" only
to be later in GNUmed discontinued -- these would remain helpfully available in
the list, providing only that the user clicks the [ ] Inactive box.

It is pretty clear that what GNUmed currently provides is not "just" a "Current
Medication List". It is a "Medication list".

--> so let us henceforth call it a "Medication List".

Comment 1b:
------------------
The use of two different words (discontinued, inactive) in the same GUI risks
confusion. "Discontinued" could mean "instructed to stop taking because it was
harmful or unhelpful or maybe not needed" or that the patient decided they will
no longer take, while "Inactive" could mean "may have it available, and may
decide to take it again".

These are related, but not the same, and yet inclusion / exclusion in the list
("Inactive" toggle) is determined solely by having a NULL or non-NULL in the
column 'discontinued'.

The problem is that the use of two different word-labels can cause any user to
wonder whether "inactive" might refer to something that is not in view or fully
enough explained in the GUI.

--> so can we make the terms consistent, and alter the checkbox name to
"Discontinued"?
