I would welcome some description of how this class of information is to be 
separately handled within GNUmed. At the conceptual and/or schema levels.

For example, is the problem list intended to be divisible or filterable, in 
terms of

        those health issues or unattributed episodes being handled by the praxis
        i.e. by the praxis doctors, whether physically inside or outside the 
praxis

vs

        those health issues or unattributed episodes being handled outside by 
the praxis
        i.e by clinicians who are not among the praxis' doctors?

??
