Ah, the joys of a pre-built package. :-)
If nobody objects, I think that's the best way to go.
The paper wasn't in the indexing tools archive; there's a gzip'ed
PostScript file at http://cm.bell-labs.com/cm/cs/cstr/128.ps.gz
though.
You don't have to enclose anything in quotes, as far as I can tell:
just use .ix writing macros and so forth. The CSTR 128 paper
says it doesn't support multi-level index entries, but looking through
the README and so forth, it looks like that might have been added.
Time to start claiming chapters!
