Building groff-1.22.2 from the release tarball on OpenBSD,
i do have the two lines
Making gropdf.n from gropdf.man
Making pdfmom.n from pdfmom.man
in the build log. So i still suspect that the Gentoo groff-1.22.2.ebuild
script somehow uses the groff build system incorrectly.

I strongly suspect that this analysis is somehow flawed, though
lacking detail, it's hard to say in which way.
Do you maybe compare a groff-1.22.2.ebuild controlled build of
the groff-1.22.2 release with a vanilla build from a git master
HEAD checkout, the latter *not* using groff-1.22.2.ebuild?
