Details:

We need to differentiate the evaluations / encounters :

* Urgency Level

* Location : Ambulatory, Inpatient
-> If it's an inpatient evaluation, the inpatient/hospitalization code will be
requires. Inpatient evaluations should be generated from the patient
hospitalization main form (relate).

* Occurrence : New, folllow-up, referral 
-> If follow-up, then we need to relate it to an existing evaluation.
 
* Media : On-site, telephone, telemedicine
