The /usr/local/www/hdl/htdocs/includes directory is a hard-coded default in case
there is no "includes" directive in /etc/koha.conf. However, installer.pl does
not set up the "includes" directory to point to the correct directory. As a
result, startmenu() falls back to the hard-coded default.

After adding a correct "includes" line in /etc/koha.conf, the categores page 
shows.

Need to fix installer.pl to set up the "includes" line.
