The primary place I tend to see these sorts of things happen is when someone
is using a Mac or a Win32 based application and the software is determined
to use some special character for quotes - instead of using " , ', or - they
use 'smart quotes' and other specials. The Mac is notorious for this -
using the 'true' left and right quotes instead of the lower ASCII ones.
