Details - The buffer type (pdf_buffer_t) is a handy way of managing buffers in 
the library, but it is only used internally. Also, the pdf_buffer_t is not an 
opaque type, and not only that, the read-pointer and write-pointer elements 
need to be updated externally, not with any method, directly modifying the 
struct members.

I suggest to make this type private to the library and remove it from the 
API... comments?
