I have good news from the OS X front: The reason why plugins in  
$HOME/.lv2 do not work anymore is that SLV2 now scans the following  
directories:

$HOME/Library/Audio/Plug-Ins/LV2 (only if $HOME is defined)
/Library/Audio/Plug-Ins/LV2
/usr/local/lib/lv2
/usr/lib/lv2

So everything seems to be sorted out now and you can copy the plugins  
to any of these locations.
There is a new disk image with updated installation instructions

Remon, may I ask you to place it on the website, too?

Those who have problems with messed-up toolbars, please try the  
following image:
