The problem is something to do with the interaction of
width { expand } and running headers. I've found that
by deleting width { expand } you can get it to work, so
I suggest that as your immediate work-around. Meanwhile
I will look into the problem and incorporate the result
(whatever it turns out to be) in the next release.
