Glossary and index

Hi,

There was a problem reported some time ago with glossaries when used in
conjunction with appendices.  Jeff Kingston had a solution which
consisted in moving the lines that call @GlossaryPart in bookf
(type "lout @MakeGlossary" in Google groups to find it).

However, there are also layout problems when both @MakeGlossary and
@MakeIndex are set to "Yes".  Without the patch, @GlossaryWord appears
on a blank page, followed by the glossary entries on the next page.
Using this patch, the glossary is laid out properly but @IndexWord
gets printed on a blank page following the index entries page.

Any idea?
