In care2x, it's a matter of navigating the links until you find where you can put some *notes* under the patient record. Maybe it records the time and date of notes for you. Time , place, patient for a *note* and that's it for the doctor's side of things: The *note* maker in the vast hospital information machinery or care2x ;)
