There isn't really an explicit notion of "set". If I enter
SOAP notes under one and the same encounter for one and the
same episode they end up as one combined list of soap rows
on equal footing with each other.

Now, within one encounter SOAP notes *can* be
aggregated/filtered by episode or soap category. They also
*can* be grouped by insert time but the latter grouping is
just by coincidence and not explicitely modelled. There is
no idea of "entry session" within an encounter apart from
such rows accidentally sharing the insert timestamp courtesy
of them being added within one transaction -- just an
implementation detail not guarantueed to persist in later
versions. In fact, with the advent of editable notes, that
coincidence is bound to be breakable.
