I think I have similar need.
Try to specify some details what you want. Then maybe I will create something
that will suit both of us, and perhaps others too.
If you can do it in Launchpad, we can test how it works there. (I think it
should be submited as blueprint.)

Ok so let´s say I consult Mr. Fred Fredericks
At the time he is to leave I want to be able to print a Report with:

and the discussion is summarized below.
- Patient Request -
- Subjective -
-Objective -
- Assesment -
- Plan -
Today, <date> signed by <doc>

Other case scenario is patient comes to a visit, or more than one, and then his other doctor asks him "what is going on with that Acupuncture guy you are going to?"
So I need to make a Summary of the consultations, or at least a Smmary of last X-months encounters:

Mr. Fred Fredericks in <date> asked <doc> to make a Report about the current standing of his consultations.
This Report spans from <date wished> up to today.
-Healt Issues discussed up to now -
<the list of Health Issues>
- Consultations on each Health Issue -
<Health Issue>
- <episodes for each health issue>
- - <encounter for each episode>
- - - <RFE> <Subjective> <Objective> <Assesment> and <Plan> for each encounter.
