[Gnumed-devel] improved tooltip for lab results

Note how the lab name and contact (here: phone number) is
pulled in. This is specific to the lab the result came from
(if available) thus making for convenient calling the lab on
behalf of that result/ordering follow-on tests etc.

I have since moved it below result such that the sequence is:

 date
 type
 result
 source
