If anyone wants to do something XMPP related he/she could explore the data 
exchange issue. To be ahead of the pack all FOSS EMR should implement easy 
patient data exchange.

Your job would be:
1.) identify all open and closed standards for exchange file formats ( CDA, 
xDT for Germany)
2.) identify all open and closed data exchange protocols (HL7, HXP [1])
3.) identify all open and closed data exchange software packages (mirth, XMPP)
4.) identify and evaluate FOSS XMPP clients and libraries(preferably in 
python)

4.5) Talk to the XMPP guys to find out if they let you do it under their 
umbrella


5.) hack a XMPP client to select data exchange partner (like a contact from a 
roster)
6.) hack a GNUmed to export data in standard format, send it to the selected 
partner
7.) interface with medical idetity provider (e.g. Docceck for Germany) to 
allow sending messages only to verified parties.
8.) document your data exchange library
9.) Optional: talk to other FOSS or non-FOSS vendors to implement it.
