Actually, the staff should have access to the non-SOAP (non-clinical, non U) 
rows, in order to better know what other entries or revisions the other staff 
had made about a patient's administrative interactions, such as

>       'Mrs Smith is not coming today, she has the flu' or
>       'Mrs Smith is not coming today, she has been admitted to XYZ hospital'


Therefore, the front desk staff's versions of the Notes plug in (the wxGlade 
panel that it calls) could:

        - at right, offer the proposed single Notes field (which would be 
stored with soap_cat = null)

        - at left

                - in left upper, omit the problem list (as already proposed) but
                - in left lower, retain the notes, and simply filter these to 
display only where soap_cat = null
