This is a re-post of what has been posted by a Florida-based physician here:

It goes to the question of how one might substructure notes.

It further appears that some subitems want free-text responses and others categorical (with free text detail) such as shown with Frequency.

I'll have to look into it more during some Christmas break, but it looks interesting. Anything to avoid bloat, which is what boiler-plate text entries risk.
