Based on Hal's advice, I killed gpsd, and asked gpsmon to look at /dev/ttyS0.

I can see a value for PPS Offset, as will as the -----PPS---- lines.
 
This is with gpsmon pointed directly at the device (gpsmon
/dev/ttyUSB0), not indirecting through the daemon. When going through
the daemon you won't get PPS packets in gpsmon; superraw mode doesn't
pass them through.  I should add that caveat to the manual page.

Please, please, do.  I think I wasted your Friday evening due to this.
 
I think the smoking gun is the absence of "PPS edge" messages in Sanjeev's
gpsd log messages.  Edge detection is failing for him; Gary, is it for you?

gpsd MUST be seeing some edge, as it passes it on the ntpd.

I think:
  1. gpsd should pass this to gpsmon as well (in an ideal world)
  2. If that is not possible, then we need a clear waiver in the gpsmon documentation, and gpsmon should show "Not Available" in the "PPS Offset:", not leave that blank
