Well, to answer my own question, I should have processed the file the
first time as:

groff -mm -z -rRef=1 dummy.mm 2> catrefs.qrf

and the second time simply as:

groff -mm dummy.mm > dummy.ps

then everything works fine.  And yes, cross references to footnote numbers
seem to work.

I would still like to know about the `modularity' of the macro sets.

Sorry to have posted prematurely.  I hope this will save someone else a
wasted evening one day!
