Details:

>From Andrew Gledhill email, we need to include the infusion rate of the
medicaments, extensively used in inpatient settings.

Please see Andrew's proposed workflow.

If we use a "infusion" checkbox, we would have the rate. The rest of the
fields exist already in the medication model.
