[Pan-users] Reporting a Bug

I cannot seem to report the following bug using the bug tracking system: 
Can anyone help please?

User Information:

Had just created a family note, OKed it so that it was listed as the 
second family note in the 'Family:' dialogue.

Clicking OK to save the data and dismiss the dialogue crashed the program 
(I did it 3 times to check)
