gmDemographics.sql:
person -> identity
job == doctor -> occupation
person == staff -> staff
staff == doctor -> staff_role
staff type == surgeon -> missing, maybe staff_role ?
doctor number == xxxxx -> person_ext_id
Certainly, the doctor status isn't being defined in the
table identity !?!
We should really rename identity to person. But that'd be a
big refactoring mess. I'd do it, though.
