I did find the hyphens & signs ( - -) a bit confusing.
Has the following any advantage for the display text?

NULL:  not asked
-1:         asked, doesn't know, collateral lacking / inadequate (this 
is long, I know)
0:          asked, no allergies
1:          asked, has allergies
