Right. There may only ever be one single row in clin_diag
referring to any single row in clin_narrative. You cannot have
two clin_diag rows referring to the same soAp row and one
saying "left" while the other says "right".

It doesn't *code* the clin_narrative row at all. It gives
additional information about the soAp row making it a
diagnosis. And yes, you can only once give additional detail
about any single soAp row.
