Well, you have reasonable chances to mix distributions - but the
price is that the system is not "stable" any more.  I have no
experiences with mixed Etch - testing systems - it might be that
you need a lot of packages from testing which might make the
approach questionably and running a complete Lenny which is
"quite" stable these days might be a reasonable option.

However, 0.2.8.x can be run from a tarball just fine
in Debian/Stable.

This could be a sensible alternative.

If this is explained I will put it somewhere in the wiki because it may
help to be clear how people with a stable system (even when lenny is
stable) may need to be able to update their gnumed through some means
other than using cvs

They can't. The stable system is stable because it doesn't change.

Well, the third option is to try backports.org.  I know several
people who are using this - but I have no personal experience.
May be adding this to your sources.list and add a proper pinning
statement might enable you to use gnumed-client packages from
testing - but I have not tried this personally ...
