GNUmed would not guess, it would execute user expectations
(except that you don't have this expectation - which is
exactly why I am asking).

GNUmed would display the same codes for each COPD episode,
courtesy of the user having associated codes with the term
"COPD" *anywhere* previously. Conversely, if the association
is modified anywhere it will display as modified everywhere,
automatically. This would happen courtesy of the table
structure without GNUmed doing anything specific about it.
