I (maybe) see… looking further at (for example)

        clin.lnk_code2episode

this inherits most of its structure from the root link table, beyond which the 
child table clin.lnk_code2episode:

- also grows a pk, which makes sense
- has its fk_item column description set to 'Foreign key to clin.episode'
- has its F-Key column value (for fk_item) is clin.episode.pk which makes sense

However this link table is not referenced by other tables ??

Therefore, it depends on the application to create these links between:

        a row in a particular patient's record and
        (via this link table's
                fk_generic_code --> ref.coding_system_root.pk_coding_system )
        a row in ref.coding_system_root 

The purpose being that ?? it is easier to check existence of whether a code has 
been used by a patient if the query can simply check

        clin.lnk_code2item_root

instead of having to check all of the individual lnk_code2... tables?
