[Gnumed-devel] simplifying gmdrugs.sql

I had a few complaints about the complexity of gmdrugs.sql, and I am starting 
having my own concerns regarding performance with so many subqueries 
neccessary.

I suggest denormalizing and simplifying where appropriate.
My suggestion regarding just the drug names, drug elements and ATC codes is 
attached for discussion.

Since the changes would be significant, I did not want to submit without prior 
discussion
