If LDT (and therefore the source of your results) uses no coding system, do you rely on the fact that the lab *surely* (??) uses some arbitrary labelling system ("code") of its own e.g. haemoglobin may be "Hgb", haematocrit may be "Hct" and it is with these that you populate test_type for that particular test_org?
And so in test_type is the value for coding_system kept "NULL"? PS in the table test_result the field id should be renamed pk (better now, than later ;-))
