I think the SOAP widget interface is quite good ; it's got
a certain inflexibility that tries to make users think logically
before entering an encounter.
There's a few functions I can't find or need to be done
I think for 0.1; e.g. health issue creation , reassignment of 
unnamed episodes to other health issues,  allergies editing,
addressbook; request/ref generation ( using the test-area/ian stuff);
I still don't know how to , say, browse a directory of image files ,
and attach them to specific people by patient search and manual linking.
Current medication and script management, even if it's just a locale
specific implementation, and decision support only for say
20 important common drugs,  would be good for 0.1. 
Currently, I'd like to try to get a basic hl7 v2 mapping to lab_request; 
test_result
sub-schema for 0.1 as well.
Too much to ask for?
