I should think we can further narrow down this part: IMO it
should be sufficient for GNUmed to *select* the items to put
on the invoice but NOT to create anew such items. IOW, if
there is no item "bill for Tet immunization" inside
lx-office GNUmed won't be able to bill it and that's OK.

Make that "and". Handle the invoice *and* hand over a copy
PDF.

This is - IMO - the most promising approach for
first-iteration billing in GNUmed. We surely need to
document this in the Wiki so I (or someone else) can
eventually start work from the accumulated wisdom.
