That depends on how the system is set up.

direct linking:

        - there is no *explicit* local-term-to-code mapping table
        - GNUmed would search the *link* tables, however, to make
          decent suggestions beyond the official term
        - this scenario ("modify list of terms with their codes")
          will simply not exist - it is either "modify the patient's
          list of codes for this episode", modification of the
          official terms will not be allowed

indirect match

        - only here does this scenario apply, and then "term"
          would mean "locally generated term", not "official term"
        - the official terms would *still* exist in the reference
          tables and, again, they would not be editable

No, this is entirely independant.
