It is possible that a praxis could receive sets of org data from different 
sources:

- a data set of pharmacy contact information, from a pharmacists' consortium

- a data set of doctor contact information, from a doctors' organization

- a data set of public hospital / facility information, from government

Accordingly, can we have a nullable fk_source in

        dem.org

?
