When

        - it's known to be higher than normal in this patient
(one could define a target range, yes)

        - Politics or ivory tower academics define normal ranges
to which labs slavishly adhere (for fear of
litigation) but which are entirely irrelevant to many
          patients

Cholesterol is perhaps not the best example. Think GGT or
any number of entirely irrelevant tests which are ordered by
irresponsible clinicians anyway.
