Linux 'pass' backed by a git server, which slightly inconveniently
allows me to share password stores with family members who're prepared
to open a terminal window. Uses gpg as the encryption mechanism, and
you can add multiple keys to a store so all users use their own personal
public key pair to get in. pass has a plugin to do 2FA and will store
any textual content you can throw at it. I do not share passwords with
my 'phone, but I believe that pass does have mobile clients.

Less technically-inclined partner uses Schneier's Password Safe (nice
Windows and Android apps, and Schneier's possibly the only third party
I'd trust for something like this), and shares it between devices using
Google Drive.
