I am not sure I see the compelling need. But who am I ?

OTOH there already are a few which don't work even now (eg.
nearly all of them except vaccs/allergies) so we can just
convert those.

OTOH yet again why not just stick to Richards well-proven
design and use the SOAP area for, well, SOAP notes ?
